If convenient, you may complete the application form below and forward it to us via e-mail or fax. Alternatively, you may bring it with you to meet with us in person for a free consultation. When coming to meet with us, you should also bring any documents regarding your debts (i.e.: credit card statements, loan statements), ownerships to vehicles, trailers etc., documents regarding any RRSPs or pensions, life insurance policies (other than through your employment), most recent pay stubs and your last tax return. The information you give us will assist us in providing you with solutions to your financial challenges.
This format will allow you to download onto your computer, complete it on your computer, save and e-mail it to us — or complete, print and fax to us — or complete, print and bring with you to a meeting.

This format will not allow you to complete on your computer but you will be able to open or download and print out for completion by hand. It may then be scanned and e-mailed, faxed or brought with you to a meeting.
